Launching Health Commodity Active Distribution in Burundi

The Central Medical Store in Burundi (CAMEBU), in collaboration with national programs, transitioned from an exclusive pull distribution system to a combination of pull and push distribution systems with the help of the USAID Global Health Supply Chain Program - Procurement and Supply Management (GHSC-PSM) project. This new distribution strategy came along with the introduction of multi-month distribution (MMD) of antiretrovirals (ARVs), mainly with the TLD90 and nevirapine-based combinations phase out. Its primary goals are to optimize storage conditions, improve transportation conditions and ensure the availability of health commodities at all levels of the supply chain, in conjunction with a COVID-19 prevention strategy based on ARV multi-month provisions and a reduction in patient visits to health facilities.

The pull distribution occurs monthly for a specific category of commodities that require monthly dispensing of treatments to patients, whereas the push or active distribution occurs quarterly for commodities that require multi-month dispensing, particularly ARVs.

The first active distribution was launched in September 2020 and piloted in 11 health districts. After three rounds of active distributions, CAMEBU scaled-up the strategy to all the 48 health districts of the country.

With the assistance of USAID through GHSC-PSM, quarterly quality monitoring, re-evaluation and improvement of the active distribution strategy were organized so that any bottlenecks that arise can be resolved quickly, without disrupting the system.

CAMEBU, in collaboration with national programs and GHSC-PSM, organized a workshop in December 2021 to assess the active distribution strategy, evaluate the findings, discuss the flaws and make changes to improve the system.

To date, with the 5th round of active distribution, GHSC-PSM has assisted CAMEBU in distributing 107,325 Artemether/Lumefantrine 120/20 mg treatments and 83,686 TLD90 to manage HIV and malaria case rates while reducing mortality for children and adults, with more than 80% of these patients receiving ARV multi-month provisions.

The overall goal of GHSC-PSM in Burundi is to strengthen key government institutions and create long-term systems to ensure the availability and security of vital health commodities for the people of Burundi.

To date, GHSC-PSM has assisted CAMEBU in distributing 107,325 Artemether/Lumefantrine 120/20 mg treatments and 83,686 TLD90 to manage HIV and malaria case rates and reduce mortality for children and adults in Burundi.
